The Board sent letters to all eligible Diplomates, which includes Diplomates who hold life-time certificates and time limited certificates ending in 2022. Letters was also sent to Diplomates whose certificates expires in 2018, 2019, 2020 and 2021 that did not go through the process in 2013, 2014, 2015 and 2016 or did not successfully complete it. The letter provides instructions on how to complete the MOC 5-year 2017 application.

There is a $500.00 fee to submit the MOC 5-year 2017 application. The application deadline is September 15, 2017. An administrative fee of $50 will be charged if the application contains errors or omissions. A $350 late fee will be charged for any application received from September 16- October 2, 2017.

As part of the application process, Diplomates who are Certified-Active will be expected to upload the following documents or complete the following information on the application:

  1. All medical licenses (PDF format);
  2. One letter of reference from the VP of Medical Affairs on an institutional letterhead with a signature (PDF format); OR use the reference template form.

  1. One letter of reference from a responsible member on staff on an institutional letterhead with a signature (PDF format); OR use reference template form.
  2. A practice profile; 
  3. A list of 150 AMA Category 1 CME hours since January 1, 2013, half (75 hours) of which need to be in a broad category of thoracic surgery; and
  4. The name and explanation of a Practice Quality Improvement (PQI) project completed in the past 2 years.
  5. The name and explanation of a Patient Safety Course.

Diplomates who are Certified-Inactive will be expected to upload the following documents:

  1. All medical licenses (PDF format);
  2. An explanation describing what you are doing professionally; and
  3. A list of 150 AMA Category 1 CME hours since January 1, 2013, half (75 hours) of which need to be in the broad category of thoracic surgery.

If selected for random audit, Diplomates will be expected to upload copies of the third-party CME certificates (PDF format).

A coupon code for access to the most current SESATS will be sent sometime after the late application deadline.
