10-year Milestone for 2017

As part of the application process, Diplomates who are Certified-Active will be expected to electronically submit the following documents or complete the following information on the application.

 

  1. All medical licenses (PDF format);
  2. One letter of reference (Letter Template) from the Chief of Staff OR on an institutional letterhead with a signature (PDF format);
  3. One letter of reference (Letter Template) from a responsible member/colleague on staff  OR on an institutional letterhead with a signature (PDF format); You can use the template or provide a letter on institutional letterhead.
  4. A practice profile;
  5. A summary of the last year of cases;
  6. The name and brief description of a Practice Quality Improvement (PQI) project performed in the last two years;
  7. The name of a Patient Safety Course taken in the last year; and 
  8. A list of 150 AMA Category 1 CME hours since January 1, 2013, half (75 hours) of which need to be in the broad category of thoracic surgery.

Diplomates who are Certified-Inactive will be expected to submit the following documents:
  1. All medical licenses (PDF format);
  2. An explanation describing what you are doing professionally; and
  3. A list of 150 AMA Category 1 CME hours since January 1, 2013, half (75 hours) of which need to be in the broad category of thoracic surgery.

If selected for random audit, Diplomates will be expected to submit copies of the third-party CME certificates (PDF format).

 

Diplomates must take and pass a secure MOC online exam that will test fundamental and practice-related knowledge. Diplomates may take the exam starting in the eighth year but must pass it by their tenth year. Instructions will be provided once your MOC application has been approved.
